Refer a Patient
At Providence, we are committed to making the referral process as efficient as possible. The following information is for health care professionals. Physician referral is required.
- Inpatient Rehab Referrals: For individuals who require an inpatient rehab stay in our Hospital with the primary goal to improve and optimize their functional independence in order to return home safely.
- Palliative Care Referrals: Palliative Care serves predominantly older adults experiencing the end stage of symptoms of a terminal illness and life expectancy of greater than 30 days.
- Outpatient Clinics and Services Referrals: For individuals who do not require an inpatient rehab stay, for those who have returned home after completing our inpatient rehab program and/or for those individuals at home in the community. The outpatient clinics and services are designed to promote recovery while living at home.
- Ambulatory Services: We offer a number of clinics and services for the community through Providence Hospital. Referrals are specific to the service.
