When You Arrive

When you arrive at Providence, our Admissions team will meet you and fill out any relevant paperwork. You will then meet your Palliative Care team members, an interprofessional group that includes a Physician, Registered Nurse, Social Worker, Pharmacist, Dietitian and Chaplin. Other team members that may be involved with your care include a Physiotherapist, Occupational Therapist, Speech Language Pathologist and Therapeutic Recreationist.

What do you need to bring?

Please bring the following list of items with you on your admission day:

  • Your insurance provider and policy number
  • Any decision making documentation, such as Power of Attorney documentation, living will, etc.
  • A current list of all your medications, including dosages that you take. Also include any over-the-counter drugs that you take (e.g. natural and herbal medicines)
  • The name and contact number of your Family Physician
  • Personal items that you would like to use (e.g. soap, toothbrush and toothpaste, body lotion, shoes, comfortable clothing, etc.)

Your room will have a bed, a small bedside table, a chair, a closet for your clothes and belongings, and a message board for personal memoirs, photographs, etc.
