Complex Continuing Care

Our Complex Continuing Care programs are designed for those patients who may require continuing medical management and have significant functional disabilities. We provide a mixture of active and compassionate therapies aimed at ultimately improving quality of life.

Providence’s Complex Care programs are supported by skilled interprofessional teams including Physicians, Registered Nurses, Registered Practical Nurses, Pharmacists, Occupational Therapists, Physiotherapists, Therapeutic Recreationists, Social Workers, Dietitians, Speech Language Pathologists, and Spiritual and Religious Care. All patients and family are engaged in identifying goals and ongoing care planning.

The programs are:

Complex Care

Complex Care provides ongoing medical and nursing assessment, intervention and care for adults living with progressive or severe functional disabilities and multiple medical problems.

Palliative Care

Palliative Care serves predominantly older adults experiencing the end stages of a terminal illness. Our focus is on optimizing patient comfort, supporting independence and participation in leisure and self-care activities.
