Providence Hospital is one of Ontario’s largest rehabilitation and complex continuing care hospitals. The rehabilitation program focuses on providing our patients, and their families, with the interventions, treatment, resources and support required to recover to the their maximum potential, and to achieve independence with dignity.

Rehabilitation is designed to assist adults in regaining optimum physical, mental and functional levels and includes the following programs. Specialized Stroke Rehabilitation (includes both high-tolerance and low tolerance services); Orthopaedic Rehabilitation; Amputee Rehabilitation; Specialized Geriatrics Rehabilitation (includes both high-tolerance and low-tolerance services) and Pre-Rehabilitation.

With the assistance of the interprofessional rehab team, the environment encourages and mobilizes patients to functioning levels that are optimal for their physical abilities and will enable them to resume their activities of daily living. An important part of the process is the early establishment of an expected discharge date so that patients can be discharged as soon as they are ready, to the most appropriate setting for their own individual needs.

The Complex Continuing Care Program at Providence Healthcare provides ongoing medical and nursing assessment, intervention and care for patients with severe functional disabilities and multiple medical problems. The program includes Complex Care, Palliative Care and Transitional Care for patients who are waiting for a long-term care placement.
