Providence Healthcare’s Health Information Management department oversees the collection, storage and retention of inpatient health records according to Ontario’s Personal Health Information Protection Act (PHIPA). Confidentiality of all patient/resident/client personal health information is respected and maintained at all times. The department is responsible for the release of personal health information, including providing copies of health records to authorized individuals.

How do I get a copy of my health record?
You or your legal representative must complete a consent form and either mail, fax or hand deliver it to the Health Information Management department.
The consent must be addressed to Providence Healthcare, signed, witnessed, dated and specify to whom the information is being released to.
Please Note: If you are a legal representative, we require the original signed consent form in the order to complete your request.
Is there a fee for obtaining my health record?
There is no fee when releasing information to physicians or other health care facilities for the purpose of continuity of care.
For requests other than for the purpose of continuity of care, including individuals requesting a personal copy of their record, there is an administrative fee as outlined in the fee schedule below, aligned to guidelines set by the Office of the Information and Privacy Commissioner of Ontario.

What if I disagree with the information entered in my health record?
Under PHIPA you have a right (with some exceptions) to request a correction of your own health record. In order to make a correction to your health record, please fill out the Request to Correct Personal Health Information form, clearly outlining what you are requesting to have changed.
Can I obtain the Death Certificate from Providence Healthcare?
Death Certificates must be requested at the Office of the Registrar General. Please visit the Government of Ontario website.
